Discharge-planning support
Services to line up alongside a discharge — not a bed
What this is
Guardianship & insurance-enrollment help + community supports
These are support services accessed alongside a discharge(a patient who needs a conservator, or help enrolling in Medi-Cal / Medicare before they leave) — not a destination bed, so they don't appear in the resource map. Every entry is a real, cited organization from an authoritative public page (curated-seed tier); none has a keyless live feed, so each is shown with its source + freshness and re-verified on a schedule.
Refer a family to the money layer: IHSS & VA programs that pay a family member to provide care, CalAIM community supports, Medicare Savings Programs / Extra Help, and copay assistance — cited structure + the county/VA office to call. →
The going-home directories: non-medical / private-duty home care (CDSS) and Medicare DMEPOS equipment suppliers (CMS), county-filterable with source + freshness. We surface the directory and hand off the ordering. →
A labeled community-sourcedlayer (Open Eligibility categories) for SDOH needs — county- & need-filterable. Complements the clinical map; not clinically verified. →
