These are support services accessed alongside a discharge(a patient who needs a conservator, or help enrolling in Medi-Cal / Medicare before they leave) — not a destination bed, so they don't appear in the resource map. Every entry is a real, cited organization from an authoritative public page (curated-seed tier); none has a keyless live feed, so each is shown with its source + freshness and re-verified on a schedule.

Guardianship / conservatorship
1Court-appointed Public Guardian–Conservator (Probate + LPS) for adults who cannot meet their own needs when no qualified, willing family is available.
Eligibility: Adults lacking capacity, no suitable private conservator; by court referral.
Insurance enrollment help
3Certified Enrollment Counselors help clients enroll in Medi-Cal, subsidized Covered California plans, and CalFresh; free.
Eligibility: Open; free enrollment assistance.
HICAP — free, unbiased Medicare counseling, plan selection, Medigap/long-term-care insurance, and appeals for Alameda County. Statewide HICAP line 1-800-434-0222.
Eligibility: Medicare beneficiaries & caregivers; free.

Free / loaned medical equipment
1Free loaned durable medical equipment (wheelchairs, walkers, commodes, hospital beds). Volunteer-run network since 1996; also SF + San Rafael sites.
Eligibility: Open to community; donation-based, no fee.
Community support
1Bulk surplus medical-supply + equipment REUSE NODE (clinics / global health) — NOT an individual loan closet.
Eligibility: Institutional / partner clinics, not individual patients.
