These are support services accessed alongside a discharge(a patient who needs a conservator, or help enrolling in Medi-Cal / Medicare before they leave) — not a destination bed, so they don't appear in the resource map. Guardianship / conservatorship
9Court-appointed Public Guardian–Conservator (Probate + LPS) for adults who cannot meet their own needs when no qualified, willing family is available.
Eligibility: Adults lacking capacity, no suitable private conservator; by court referral.
Conservatorship of person and/or estate when no one else is qualified and willing to act; appointed by court order. Located in Martinez (call for office address).
Eligibility: Adults lacking capacity; by court order.
Conservator of last resort (Probate + LPS) managing property, finances and personal-care needs of adults unable to provide for themselves, after other resources are exhausted.
Eligibility: Adults lacking capacity; conservator of last resort.
Conservatorship for adults unable to meet basic food/clothing/shelter needs or manage finances; arranges care, housing, health care and transportation. Referrals from agencies, courts, families, medical facilities.
